Schedule: Monday - Friday
Pay Range: $25.72 - $35.37 Depending on Years of Experience
JOB PURPOSE: The Behavioral Health Case Manager, as part of the patient care team, provides direct case management of all assigned patients; thereby contributing to the improvement of patient care, their quality of life, and treatment compliance by the patient. The Behavioral Health Case Manager serves as a liaison and advocate for the patient and family, ensuring the delivery of quality and timely services to the patient from the patient care team and collaboration with community agencies.
Essential Job Functions:
Provides supportive case management to patients actively enrolled in behavioral health services, monitoring their needs and progress.
Assists in identifying, implementing, maintaining, monitoring, and/or modifying behavioral health services.
Coordinates care activities ensuring continuity of care between levels of care and community resources.
Outreach and follow-up care after crises or missed appointments.
Participates in case staffing and meetings as needed for the patient.
Utilizes internal and external resources to support recruitment and retention of consumers.
Receives clinical oversight and supervision by a Behavioral Health Professional regularly.
Initiates referrals and supports transitions to requested services.
Provides triage and navigation to specialized behavioral health resources.
Maintains patient confidentiality and compliance with HIPAA and corporate standards.
Conducts comprehensive intake assessments and collaborates with care teams.
Completes required documentation for new patient assignments and audits patient charts.
Maintains regular contact with patients to monitor progress and modify treatment plans.
Participates as a team member to ensure appropriate care and resources.
Documents patient care accurately and timely, meeting regulatory standards.
Develops positive relationships with patients and their families.
Serves as a communication link between patients and medical/behavioral health staff.
Responds to non-appointment related calls and resolves issues.
Understands government and community social agencies for effective referrals.
Maintains information on support services for patient care.
Understands applicable Federal, State, and commercial requirements.
Maintains a clean and safe work environment.
Demonstrates understanding of compliance and reporting requirements.
Minimum Education and Experience:
Bachelor’s Degree in Social Work, Psychology, Sociology, Public Health or a related field.
One (1) year of experience in human services, behavioral health, or case management.
If applicable, equivalent combination of education and experience may be considered.
Required Licenses, Certifications, and Registrations:
Level I fingerprint clearance card: current valid and in good standing.
Current provider card in Basic Life Support (BLS) and advanced life support certifications if relevant.
Reliable transportation and valid driver’s license compliant with Arizona vehicle insurance requirements.
Preferred Education, Experience, Skills, Abilities:
Master’s Degree in relevant fields.
Three (3) years of experience in relevant fields.
Bilingual in English and Spanish.
