We have an exciting opportunity for a Case Manager to join our multidisciplinary Team at the Albany Med Center of Excellence for Alzheimer's Disease to make a significant regional impact on delivering high quality specialized care for patients with Alzheimer’s disease and other related dementias.
The Case Manager, with oversight by the Program Manager of the Center of Excellence for Alzheimer’s Disease (CEAD), will administer the grant deliverables and program evaluation components of the CEAD, as specified in the contract with the NYS Department of Health (DOH).
