Salary: $51,200

The Esperanza Center provides a welcoming atmosphere, social services, health services, legal support and English as a Second Language (ESL) to immigrants in the Baltimore metropolitan area. The center’s staff offers assistance with employment, English proficiency, legal services, and access to primary and urgent health care and referral services. Health services are offered in partnership with area hospitals and medical schools.

Catholic Charities of Baltimore Building Hope is currently seeking a Case Manager (Bilingual English/Spanish) who provides intensive case management, including, but not limited to, as‑needed home visitation services to participants/clients. The case manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services. The work schedule is Monday – Friday 8:30 am‑4:30 pm (37.5 hours per week). This is an onsite position only.

This position is supported by grant funding; employment may depend on the continuation of the funding.

Job Duties & Responsibilities

Develop Individualized Service Plans (ISPs) with participants, focusing on self‑sufficiency within the established timeframe and tracking progress.

Maintain consistent contact, conduct required visits, and address challenges to ensure participant engagement and compliance with the ISP.

Conduct self‑sufficiency assessments at intake and closure using the Arizona Self‑Sufficiency Matrix, documenting and communicating results.

Advocate for individuals, families, and children, securing needed services within the community.

Collaborate with staff and volunteers to engage clients in programs that support their path to self‑sufficiency.

Maintain accurate case files, enter data into the Service Point database promptly, and ensure data integrity for reporting.

Collect, input, and report data per program requirements and keep supervisors informed of trends and outcomes.

Maintain a working knowledge of program services and connect clients to internal resources for comprehensive support.

Perform other duties as assigned.

Education & Experience Requirements

Bachelor’s degree in Human Services, Social Work, or related field.

Two years of related experience.

Excellent knowledge of case management and crisis intervention services.

Equivalent combination of education and experience may be considered.

Proficiency in computer systems and software (Windows PC, web browsing, Microsoft Outlook, Word, Excel, PowerPoint, Teams, OneDrive).

Physical Requirements & Work Environment

No remote or hybrid work; stationary position with standing or sitting for extended periods.

Walking, standing, or traveling to various locations as required.

Verbal, listening, writing, and electronic communication with colleagues, clients, and external parties.

Repetitive motions involving wrists, hands, and fingers.

Assessment of accuracy and completeness of work to meet quality standards.

Adequate hearing and vision; driving required with valid driver’s license and at least 2 years of experience.
