Job Responsibilities:
Assist individuals in accessing medical, social, vocational, and educational services
Develop service plans
Coordinate and monitor services

Requires flexible hours, extensive paperwork, and self-directed performance. Travel and personal vehicle use are required.

Residency within a 70-mile radius of the duty site
