POSITION SUMMARY
The Case Manager meets with patients and families to explain the scope of hospice services, coordinates admission of patient and the ongoing plan of care.
ESSENTIAL FUNCTIONS OF THE JOB/RESPONSIBILITIES
Provide professional nursing care by utilizing all elements of nursing process
Assess and evaluate patient’s status by writing and initiating plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary
Develop a care plan that establishes goals, based on nursing diagnosis and incorporates hospice nursing actions. Include the patient and the family in the planning process
Administer medications and treatments as prescribed by the physician
Provide healthcare instruction to the patient as appropriate per assessment and plan
Act as Case Manager when assigned by clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload
Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate
Communicate with the physician regarding the patient’s needs and reports changes in the patient’s condition, obtains/receives physicians’ orders as needed
Communicate with community health related persons to coordinate the care plan
Provide and maintain a safe environment for the patient
Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the needs of the patient and family/caregiver
Attend interdisciplinary group meetings
Participate in on-call rotation; may assist with admits
Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis
Supervise LVN and CNA care plan and delegate responsibilities when required.
POSITION QUALIFICATIONS/REQUIREMENTS
Registered Nurse in the state of Texas
1 year of nursing experience strongly preferred
2+ years of experience in hospice case management strongly preferred
