PATH is seeking passionate candidates to join our Supportive Housing Services (SHS) team as a Case Manager at our Autumn Court site in Los Angeles, California.
About PATH Since 1984, PATH (People Assisting the Homeless) has pioneered approaches to assist people experiencing homelessness through prevention, outreach, employment, case management, and housing services across California.
Position Overview The Case Manager supports clients in breaking the cycle of homelessness using a "whatever it takes" approach, assisting through all phases from street to permanent housing.
Responsibilities Conduct intake and needs assessments, develop Individualized Service Plans (ISP), and address housing, income, and personal goals.
Provide referrals, support, and linkages to resources.
Complete assessments like VI-SPDAT and evaluate client progress.
Maintain accurate documentation and ensure confidentiality per HIPAA guidelines.
Qualifications Two years of experience in healthcare, case management, or working with vulnerable populations is preferred.
All education and experience levels are welcome.
Mandatory: employment eligibility, CPR/First Aid certification, background check, TB test, and drug screening.
Driving is an essential function: reliable transportation, valid driver’s license, proof of insurance, and ability to qualify for PATH insurance coverage are required.
What We Offer Competitive salary ($23.24 to $28.28/hr), benefits package including health coverage, paid time off, holidays, and a retirement plan.
Additional Information This is a full-time, non-exempt position based in Los Angeles, CA. Remote work is not available; work is performed on-site.
