Description Case Manager Assistant

Job Description:
The Case Manager Assistant provides support to Case Managers in coordinating and delivering services to clients.
The assistant is responsible for helping with administrative tasks, client interactions, and follow-ups to ensure that clients' needs are met effectively and efficiently.
Essential Job Functions Administrative Support:
Assist Case Managers with paperwork, data entry, and maintaining client files.
Schedule and confirm appointments for clients with service providers.
Prepare and organize client information for case reviews and meetings.
Client Interaction:
Communicate with clients to gather necessary information and provide updates on their cases.
Assist clients in understanding and accessing services available to them.
Coordination of Services:
Track clients' progress and report any issues or concerns to the Case Manager.
Team Collaboration:
Work closely with Case Managers and help distribute appointment cards every morning.
Participate in team meetings and contribute to case discussions and planning.
Qualifications Minimum Qualifications
Skills:
Strong organizational skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Empathy, patience, and a genuine desire to help others.
Education:
High school or equivalent.
Prefer Associates degree in social services or in related field.
