Responsibilities As part of a multi-disciplinary team, collaborate with the treatment team and serve as a primary point of contact for the client and service providers.
Identify immediate and ongoing barriers to access for mental health and substance use treatment; conduct functional assessments of skills and resource needs to guide interventions.
Develop or enhance stability in identified needs using client strengths; address skill and resource deficits under clinical direction (e.g., life skills, conflict resolution, self-advocacy, housing, medical care, job-seeking).
Participate in collaborative meetings with internal and external team members to ensure continuity of care and link clients with resources.
Work in settings that may include outpatient, home, or community-based environments.
Essential Functions Complete functional needs assessments and contribute to treatment plan development.
Enhance client engagement with the organization.
Coordinate care with clients, internal and external partners, and families or supports.
Facilitate individual and/or group sessions as appropriate.
Evaluate client progress and provide feedback to clients, families, and treatment teams.
Serve populations as assigned (e.g., offender-based, first episode psychosis, inpatient psychiatry, substance abuse clients).
Maintain accurate, timely documentation of services.
Operate a personal vehicle as needed to travel between offices or community sites; transport of clients may be required.
The Environment at Diversus Health is fluid; roles may evolve with business needs. Some duties outside the listed scope may be required. Flexible hours and overtime beyond 40 hours per week may be needed.
Knowledge/Skills/Abilities Bachelor’s Degree in Psychology, Sociology, Social Work, or related field required.
One year of professional or volunteer experience in behavioral health preferred.
Working knowledge of coping skills and community resources; general understanding of behavioral health and/or substance use services.
Ability to take initiative, organize work, and solve problems independently.
Excellent verbal and written communication; ability to build rapport with clients and team members.
Proficient computer skills (Microsoft Suite) and ability to type 25+ WPM for EHR input/output.
