Job Details: This role combines case management for the Ryan White program with HIV prevention skills. The Case Manager provides case management for clients living with HIV, including assessing needs, referring to resources, ensuring medical needs are met, and participating in quality improvement. The Prevention Health Specialist conducts HIV, Syphilis and Hep C testing, provides education on safe practices and harm reduction, and coordinates referrals for treatment or medical care.

Schedule: Monday - Friday; 8am–5pm
Wage Range: $24.28 to $32.19 hourly, depending on years of transferable experience and internal equity
Responsibilities Provide case management for clients living with HIV, including assessing needs, referring to community resources, ensuring medical needs are met, participating in ongoing education and quality improvement.
Maintain up-to-date patient charts, progress notes, care plans and related medical information.
Guide patients through financial assistance and other RiverStone Health services to ensure a full range of services.
Review and submit client requests for financial assistance.
Develop and maintain cooperative relationships with tribal and other local providers, agencies and organizations serving those living with HIV.
Collaborate with tribal and local agencies to determine community health needs and service availability.
Produce, maintain, and submit regular data and program reports for grant and other reporting requirements.
Participate in meetings, conferences, workshops and training to stay current on program principles and developments.
Develop, conduct, or coordinate health needs assessments and surveys; develop operational plans and policies to achieve objectives; participate in audits and QA projects.
Respond to communications courteously and timely; maintain HIPAA confidentiality and protect sensitive patient information.
Engage in emergency preparedness trainings and program quality improvement projects.
Enroll eligible clients in Ryan White program to ensure access and reduce disparities; network with local medical, dental, substance abuse and mental health providers; coordinate cross-provider services for continuity of care.
Provide face-to-face and/or telephonic case management; document current medical/diagnostic data in required systems; participate in program QI projects.
Promote HIV prevention services, testing and education within the community; coordinate Syringe Services Project participation as needed.
Qualifications Education And Experience
High School diploma or equivalent
Experience in social work or medical settings with at-risk populations
Any combination of experience and training that provides the necessary knowledge, skills and abilities
Preferred Qualifications
Bachelor’s degree in a health-related field
Experience working with underserved populations
Experience in a customer service environment
Required Certificates, Licenses, Registrations
Valid Montana driver\'s license
Appropriate vehicle insurance and a reliable vehicle for occasional day travel
On-the-job training emergency preparedness training in ICS 100, 200 and 700
