Job Description
Esperanza (Esperanza En Escalante) is seeking a full-time Case Manager to join our team. We provide transitional housing and resident-centered clinical services for unhoused military veterans. The ideal candidate wants to help veterans overcome barriers and transition to permanent housing. The case manager is responsible for managing multiple clients and their individual needs.
Duties
Meeting with residents a minimum of 1 time per week
Helping residents maintain wellness, autonomy, and attain permanent housing
Writing case notes and maintaining resident files
Creating service plans with clients and following up weekly on progress
Providing coordination with outside services for clients
Providing crisis intervention when needed
Performing room inspections
Taking Veterans out to look for apartments
Attending outreach events

Able to resolve conflicts, listen, and provide needed support
Able to use computer software, apps, the internet, and a cell phone to accomplish tasks
Have the ability to work independently and function as a team member
Have the ability to understand issues of substance abuse, disabilities, and mental illness as it pertains to homeless veterans
Requirements
Must have a bachelor's or master's degree in the social sciences
Prefer experience with veterans, disabled veterans a plus
Previous case manager experience preferred
Military background and lived experience preferred
