Our Adult Residential Programs serve adults with chemical dependency and/or are living with a mental illness at one of our four different locations in Albany County. The goal of all our programs is to help the residents manage their diagnosis and build the skills necessary to live successfully in the community.

Holt House — Adults with both mental illness and developmental disabilities that need 24/7 support

Apartment Program — Adults who live in one- or two-bedroom apartments and are ready for less structure and more independence

Cohoes Residence — Adults who have a mental health diagnosis and need 24/7 support to help control their symptoms

Recovery Residence — Men who have both a mental illness and an addiction that need 24/7 support

Position Summary
The Residential Case Manager is responsible for providing service plan development and case management to clients in the residential treatment facility.

Essential Duties and Responsibilities

Supervise activities of residents in residential treatment facilities.

Assist clients with utilizing communities' resources.

Provide all related treatment/case management services, including medication supervision and service plan development. Ensure services provided meet OMH and Medicaid regulations.

Ensure overall safety for all residents.

Assist in assessing skills, strengths and needs of all clients; teach and assist with functional skills (budgeting, maintaining cleanliness of personal hygiene, personal environment, communication, socialization, etc.).

Attend all mandatory trainings as well as annual refresher trainings (Crisis De-Escalation training, CPR/First Aid Certification, Boundaries, Medication Administration Record, etc.).

Conduct quality assurance activities, including participation in compliance residential programs.

Document delivery of treatment services; ensure that all records maintained are consistent with regulations (OMH, HIPAA, Medicaid, OMIG, etc.).

Create, develop, and implement a person-centered individualized transitional plan for clients.

Increase awareness and access to community resources.

Assist with building relationships with staff, peers and family members while promoting self-sufficiency.

Ensure clients maintain supplemental benefits provided to them by DSS, SSI, SNAP, etc.

Transporting clients to medical appointments, day program etc., flexing shifts as needed between residences.

Other tasks and duties as assigned.

Physical Requirements

Ability to stand for long periods of time

Ability to lift 20 pounds

Ability to drive a car or van safely

Minimum Qualifications

High school diploma or equivalent and four years’ experience in human services.

Must have good oral and written communication skills

Must have a clean, valid NYS Driver License.

Preferred Qualifications

Bachelor’s degree in a human services related field preferred with no experience OR Associate’s Degree in a human service-related field and two years’ experience in human services field

Schedule

Varies on the needs of the program, which may include day, evening, and/or weekend shifts and holidays. (Eligible for an additional $1.00 Shift Differential for qualified shifts)
