Overview Community Solutions is seeking a Case Manager to join our team at the Chase Center program located in Waterbury, CT. The Chase Center is an adult residential facility. In this role you will provide general case management duties including intake assessment, individual and group counseling, case record management, discharge preparation and aftercare services. You will uphold all Agency policies and procedures pertaining to the safety and security of clients, Agency property and the community. You will also be a role model to the clients while providing a safe environment, facilitating counseling, advocacy and support. The program administers assessments, monitors clients' compliance with service plan goals, and involves clients in services that address their criminogenic tendencies. Through case management, supervision, and provision of evidence-based groups, clients gain the tools and support necessary for successful community re-entry.
Schedule and Pay Schedule: Monday/Wednesday 1:00pm to 9:00pm and Tuesday/Thursday/Friday 9:00am to 5:00pm
Pay Rate: $23.84
Responsibilities Check daily for newly assigned clients.
Complete assessments within 3 business days of intake.
Facilitate feedback and creation of ISP within 5 business days of intake.
DOC case management sessions are conducted based on the client's assessment scores.
DOC clients: Submit monthly progress reports to your supervisor 4 calendar days before due date.
Home confinement clients: Collect and review phone records monthly to ensure compliance with the SOW.
Complete and submit discharge plans for BOP and DOC clients 60 days before the scheduled release date.
Prepare, plan and facilitate Transitional Skills and R&R group sessions (Wilmington, WH, HH, Chase).
Record all sessions with clients and upload recordings for quality assurance review.
Actively participate in quality assurance review sessions and apply new strategies and techniques.
Participate in PRT meetings by presenting client assessments and ISP updates, and provide and elicit peer support for client issues.
Make recommendations to the supervisor for corrective action as required and monitor on an ongoing basis.
Maintain client files and ensure compliance with Agency policy regarding confidentiality.
Assist clients with obtaining birth certificates and Social Security cards.
On a monthly basis, collect and copy client bank records to ensure they balance with financial records.
Complete incident reports as needed and submit to PD prior to leaving shift.
Register for and attend required trainings and boosters including MI, Case Management and Assessments.
Secure urine samples from same-sex clients and complete all required paperwork; monitor case load to ensure random urine and Breathalyzer tests are conducted as required per funding source.
Conduct weekly audits of all client medications to ensure medication compliance.
Conduct employment, job search and home confinement spot checks.
Conduct room searches, common area searches and perimeter searches as needed.
Conduct pat/pocket searches, facility rounds, and house counts as needed.
Assist with serving meals when assigned.
Serve as a role model to clients and staff, modeling pro-social behaviors.

Qualifications Bachelor's degree in a social science field and one year of experience in case management or a related field. Education may be substituted with experience on a year-for-year basis.
Excellent oral, written and interpersonal communication skills.
Ability to engage and develop collaborations with community agencies.
Previous assessment experience preferred.
Proficiency with Microsoft Office.
