Provide crisis intervention and management as needed.
Services range from monthly to weekly contact.
Provide services in‑person or via telehealth.
Meet clients where they are regarding mental health symptoms and treatment preferences.
Gather and complete all CCBHC data metrics.
Perform all duties outlined in the primary CM core job description.
Requirements
Bachelor’s Degree in social sciences; consideration for Criminal Justice or Education degrees.
Case Management II certification or ability to obtain after hire.
Strong commitment to allowing each client to live in normal community residences with adequate supports.
Skills to establish supportive, trusting relationships with persons with mental illnesses and/or substance abuse issues and respect client rights and treatment preferences.
Valid Oklahoma Driver License and satisfactory driving record; personal automobile required for travel.
CCBHC Model of Care
Family & Children’s Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. Working in an innovative and dynamic environment using a Certified Community Behavioral Health Clinic (CCBHC) model, we provide whole‑person care through a multidisciplinary team approach.

Greater access to social services for economic and social needs.
Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
