Position Title: Case Manager (Adoption Promotion Support Services)

Position Summary
The Case Manager provides support for children and adults participating in the APSS program by connecting them with resources, helping them navigate the adoption process, and assisting with meeting or finding resources to meet their needs while in the program.

Essential Duties and Responsibilities

Conduct intake process for newly referred children or adults.

Access community-based resources appropriate to the family’s needs.

Interface with Department of Children and Family Services, Department of Mental Health, court systems, schools, health care providers and other community liaisons.

Assist families with developing a needs/strength assessment.

Provide support and advocacy that are family-centered, community and strength-based.

Offer adoption-focused parenting education for adults pursuing adoption through the APSS program.

Document client’s strengths and needs; facilitate the development of measurable goals and objectives related to client strengths and needs.

Create a plan of activities to be accomplished with the client and document S.M.A.R.T. goals.

Facilitate a monthly parenting support group and collaborate with team members regarding its implementation and promote participation from clients.

Provide parenting education.

Follow state laws and agency guidelines regarding child abuse reporting.

Maintain weekly service provision requirements.

Participate actively in administrative and clinical supervision and meetings.

Receive feedback from children and families, community and service providers in a receptive manner.

Ensure accuracy in work and meet all deadline expectations.

Maintain consistent and regular attendance.

Follow up with supervisor in a timely manner regarding significant issues.

Demonstrate ownership of work responsibilities by incorporating feedback, taking initiative, and interacting in a professional manner within the assigned role and adhering to agency guidelines.

Maintain confidentiality as required by PLYS, licensing, HIPAA, and other regulatory agencies.

Follow and adhere to all field safety guidelines.

Qualifications

Minimum of a bachelor's degree.

Experience providing support services / direct client services in the community.

Bilingual (English / Spanish) preferred.

Experience working in programs focused on Adoption preferred.

Excellent verbal and writing skills.

Ability to communicate well with others.

Demonstrate emotional stability and effective functioning with children, adolescents and adults who may have mental or behavioral health problems.

Valid California driver’s license; current automobile insurance; clean driving record acceptable to the organization’s insurance company.

Pass a fingerprint background investigation.

Pass a TB test, Physical Exam, and Drug Screening.

Pay
Salary range: $22.50 - $31.15 / hour
