Essential Functions

A clinically competent Registered Nurse who is responsible for carrying out the hospital’s Case Management Plan.

Conducting intake assessments with patients and families.

Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

Create plan for care across the continuum, integrating patient/family preferences and values.

Ensuring that families feel sufficiently equipped and supported.

Developing individualized treatment plans that account for physical, financial, and other constraints.

Using clinical quality measures to influence positive patient health outcomes.

Training staff, developing goals to improve efficiency, evaluating treatment plans and working with practice partners to provide patient‑centered care.

Assessing new patients by gathering information, reviewing diagnoses and analyzing medical test results.

Educates staff regarding effective allocation of the Hospital’s resources while encouraging provision of high‑quality patient care.

Assesses, plans, implements, coordinates, monitors, and evaluates options to facilitate the continuum of care.

Monitor patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.

Advocate for resources and removal of barriers.

Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.

Act as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post‑discharge care.

This position does require on‑call times shared between two other nurses.

This position is a full‑time office position with occasional shifts as a floor nurse.

Knowledge and Skills
Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a direct patient care setting. Strong organizational and time management skills and the ability to prioritize responsibilities. Ability to make presentations before top management, public groups, and staff. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to read, analyze, and interpret journals, financial reports, and legal documents. Advanced understanding of use of electronic health record systems and electronic quality management tools.

Work Environment
The employee is frequently exposed to a variety of patient conditions and elements. The noise level is moderate.

Physical Demands
This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, crawling and climbing all day. The employee must frequently lift or move objects and patients weighing over 20 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Position Type / Expected Hours of Work
The hospital is open 24 hours a day, 365 days a year. Must be able to work based on facility needs. Must be available on an “on‑call” basis and readily available to be summoned to the facility in an emergency and is expected to report to the facility in an emergency.

Travel
Some travel for marketing, education, and training purposes is required.

Required Education and Experience
Must possess a current Kansas license as a Registered Nurse and have graduated from an accredited school of nursing. Five years of professional nursing experience in direct patient care and a minimum of three years of experience in nursing administration or a comparable management position. Must possess and demonstrate a high degree of leadership, organizational ability, communication skills, and the ability to effectively manage the provision of nursing care and quality assurance. Knowledge of nursing home management, policies, budgeting, personnel management and census development is required. Current CPR and ACLS certification required. BSN preferred.
