Case Manager-ACT – ElevaCare
Under the supervision of the ACT Team Lead/Mental Health Professional, the Case Manager supports individuals with serious and persistent mental illness in developing and maintaining stable living, working, and social situations. The role includes coordinating and monitoring individual treatment plans, evaluating mental health symptoms, providing skills training, assisting with daily living activities, promoting social connections, and delivering stage‑based treatment in office and community settings. Evening and weekend work may be required.

Essential Functions

Coordinate and monitor services for assigned clients in alignment with individual treatment plans.

Evaluate client mental health symptoms, functioning, and response to treatment; participate in developing Individual Treatment Plans (ITPs) and provide discipline‑specific interventions.

Deliver skills training, supervision, and environmental adaptations to support daily living, community placement, and quality of life.

Support clients in maintaining family and social relationships to reduce isolation.

Provide comprehensive Individual Dual Diagnosis Treatment in collaboration with the ACT Team to reduce psychiatric symptoms.

Facilitate individual and group treatment in office and community settings using a stage‑based, client‑centered approach.

Maintain professionalism, model teamwork, and demonstrate support for the program’s Vision, Mission, and Values.

Participate in ongoing professional development to maintain and enhance community‑based mental health skills.

Perform other duties as assigned.

Other Responsibilities

Travel as needed to support client services.

Assist with data collection and program evaluation as requested.

Physical Requirements

Regularly required to sit, use hands, talk, and hear; frequently walk; occasionally stand and reach.

Ability to lift/move up to 10 pounds regularly, 20 pounds occasionally.

Required vision abilities: close, distance, color, peripheral, depth perception, and ability to adjust focus.

Education, Experience & Skills Required

Bachelor’s Degree

Must be 21+ and meet Minnesota DHS requirements for a Mental Health Practitioner.

Valid Driver’s License and auto insurance required.

Experience working with adults with serious and persistent mental illness.

Strong communication, problem‑solving, and critical‑thinking skills.

Ability to manage multiple tasks and use Windows‑based computer systems.
