Description
JOB PURPOSE: The case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards. Monitors assigned caseload of clients participating in a short-term life management program; establishes program plan/goals and evaluates client’s progress by conducting routine meetings with client and/or staff; serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment. The Case Manager is responsible for intake, coordinated assessment, individualize housing plan, and making appropriate community referrals for additional services. The Case Manager maintains appropriate contact with the client and ensures quality services are provided and that the individualized housing plan is completed timely.

Essential Job Responsibilities

Coordinate referrals, service planning and documentation of services for assigned caseload.

Interview applicants to determine eligibility for program enrollment based on established guidelines and requirements; conduct needs assessment, obtain pertinent information and establish program goals for eligible clients; refer applicants to other agencies if not appropriate for program.

Develop client intake and case plans; instruct clients in completing necessary paperwork including goals and/or budget forms; complete orientation check‑off form and obtain release of information forms when appropriate; complete social history on new clients each week.

Meet regularly with clients to discuss and evaluate their progress; prepare accurate and up‑to‑date records documenting the same; monitor client behavior, group attendance, medication and movement; receive incident reports and enforce disciplinary actions for infractions of house guidelines. Plan, coordinate, and/or facilitate life‑management classes to assist clients in obtaining skills that will enable them to functionally cope with their environment; obtain speakers and workshop presenters as needed.

Consistently serve as a mentor and provide positive examples of proper hygiene, personal appearance, records retention, budget planning, scheduling, etc.

Refer clients to appropriate programs or other agencies based on clients’ needs and in accordance with their program plan.

Complete discharge plans for clients exiting the program; transfer file contents to non‑active file.

Meet all deadlines required by program supervisor and partners.

Meet all federal and state regulatory guidelines and standards that are applicable to this position.

Maintain a caseload in accordance with agency policies and procedures contract standards.

Participate in workshops, seminars, education programs and other activities that promote professional growth and development.

Maintain a minimum number of weekly contacts with clients.

Engage and involve client in the casework process.

Continually assess ongoing changes in behavior, circumstances or conditions that may affect client safety.

Compile, prepare, submit and maintain accurate records, files, forms, statistics and additional information in accordance with agency policies, licensing and/or contract requirements.

Participate in weekly case staffing with case managers.

Maintain confidentiality in all areas of the service population and program operations.

Education & Experience
Education: Bachelor’s in Social Work, Psychology, Criminal Justice, Sociology or related field of study.

Experience: 1+ year related experience and/or training including working in crisis intervention.

Language: Proficiency in Spanish/English (written and spoken) required.

Attendance & Licenses
Must maintain regular and acceptable attendance at a level determined by the employer’s sole discretion.

Driver’s License with a clear record.

Vehicle
Must have daily use of a vehicle without prior notice.

Other Requirements
Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must not pose a direct threat or significant risk of substantial harm to the safety or health of self or others.
