Requirements
Bachelor’s Degree in Behavioral Science (or related field)
At least one (1) year of full‑time experience after completing the education requirements.
Experience working directly with adults in a human service setting, or working directly with individuals under the age of 21 in a human service setting.
Valid/current driver’s license.
Ability to travel up to 15% (may be required).
CADC license (required).
Responsibilities
Provide case management and care coordination services for patients enrolled in treatment.
Coordinate continuum of care activities for assigned patients, ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources.
Coordinate patient assessments, treatment planning, crisis intervention services, and linkages to community resources.
Monitor service delivery; assist patients in obtaining necessary services such as financial assistance, housing, food, medical, educational, vocational, and recreational services.
Assist patients with self‑help support groups, social support networks, and daily living activities; coordinate criminal justice services when needed.
Conduct individual and family educational sessions and coordinate treatment and discharge activities.
Collaborate with team members and maintain confidentiality in accordance with organizational policies.
Attend team meetings and complete required training courses in a timely manner.
Perform other duties as assigned.
Preferred Experience
1 year(s) of case management experience.
Qualifications
Behavioral Neuroscience or related field – Bachelor’s or better.
CADC license.
