Responsibilities Provide case management for consumers to access services and supports.
Assess needs, develop service plans, and coordinate with community resources.
Document assessments, progress notes, and outcomes in client records.
Qualifications Licensed Social Worker (LSW, LCSW, or equivalent) or other applicable licensure.
Strong communication, empathy, and collaborative problem-solving skills.
Ability to work with diverse populations and maintain client confidentiality.

We are an independent, private, nonprofit corporation that plans and funds services for older persons in Champaign, Clark, Darke, Greene, Logan, Miami, Montgomery, Preble, and Shelby Counties.
