This is a 3 month temporary position with possible extension. Organization: Brilliant Corners. Breaking Barriers is a rapid rehousing program serving adults on felony probation in Los Angeles County and experiencing homelessness. The organization provides housing-related services, case management, and employment supports in partnership with community organizations.

Position Summary The Breaking Barriers Case Manager will provide intensive case management services to a caseload of 25–30 program participants. The Case Manager will assist clients to reach identified goals and connect them to community resources and services. The role requires collaboration with the Breaking Barriers housing and employment teams to support clients toward self-sufficiency and a client-centered, flexible approach.
Position Responsibilities Process and accept referrals from Probation
Conduct assessments and re-assessments, including a comprehensive psychosocial assessment, to determine eligibility and needs
Develop and implement individualized case management service plans to ensure housing stability and identify goals
Support the housing process (temporary housing placement, unit identification, application submissions, lease signing, and ongoing retention)
Meet with clients regularly in home, community, and office settings
Use evidence-based practices (including Cognitive Behavioral Interventions and Motivational Interviewing) to address client needs
Provide crisis intervention and ensure access to 24/7 crisis line
Connect clients to community resources and services (employment services, health, substance use treatment, benefits, education/vocational programs, parenting programs, etc.)
Provide life skills education (budgeting, apartment upkeep, stress management, conflict resolution)
Participate in case conferences and collaborate with Breaking Barriers partners
Build partnerships with community organizations, service providers, landlords, and public entities to ensure access to comprehensive services
Provide aftercare case management for 3 months post-program
Maintain updated client files and documentation; participate in weekly staff meetings
Requirements Professional Experience
At least 2 years of experience providing direct mental health and/or intensive case management services to vulnerable populations OR at least one year of experience with a Bachelor's Degree in Social Work, Psychology, Sociology, or a related field
Experience working with individuals affected by the criminal justice system and/or homelessness is a plus
Knowledge, Skills, And Abilities
Strong interpersonal and client engagement skills
Excellent verbal and written communication skills
Flexibility in scheduling and prioritizing tasks
Willingness to travel and spend significant time in the field
Basic computer knowledge (MS Word and Excel)
Familiarity with Harm Reduction, Trauma-Informed Care, Motivational Interviewing
Ability to work independently and apply critical thinking
Good organizational and record-keeping skills
Valid, clean California driver’s license and personal insured vehicle required

Valid, clean CA driver’s license and personal insured vehicle are required
Travel Requirements
This position requires frequent visits to properties, on-site meetings, and events in the greater Los Angeles County. Occasional travel outside Los Angeles County may be required.
Labor Union
This position is part of the OPEIU, LOCAL 30, AFL-CIO bargaining unit. As part of the union, the incumbent is subject to terms in the collective bargaining agreement.
Physical Requirements
Physical mobility for standing, bending, stooping, kneeling, crouching, reaching, twisting, and walking on uneven surfaces. Ability to sit for up to 6–8 hours and lift up to 15 pounds. Requires mental acuity for analytical reasoning and documentation.
