Position Case Manager – Full Time, M-F Days.
Role The Case Manager plays a vital role in supporting patients as they transition from inpatient treatment to ongoing care. This position focuses on aftercare planning and coordination, working closely with patients, families, and providers to ensure a seamless and individualized continuum of care. Responsibilities include developing comprehensive discharge plans, securing appropriate treatment resources (outpatient providers and transitional housing), and facilitating communication with external referents. The Case Manager also provides guidance around insurance navigation, FMLA, and financial assistance as part of a holistic support approach to each patient\'s recovery journey.
Responsibilities Meet with active patients regularly for discharge planning to develop continuing care plans and ensure continuity of care
Research aftercare treatment options for patients including transitional housing if needed
Locate and secure transitional housing if needed and 12 Step meetings, specialists, doctors, therapists, psychologists, MAT providers, etc. in patients’ home areas as needed
Collaborate with families and referents on patients’ discharge plans with permission of the patient
Consult with patients and families on payment options and available financial support to create a clear understanding of the process
Assist patients and families with understanding FMLA and short-term disability benefits, focusing on guidance and support throughout the process
Help patients complete FMLA and disability-related paperwork, providing guidance on claim submission and follow-up as needed
Collaborate with the quality department to send any required paperwork to aftercare resources
Develop a strong, ongoing relationship with patients and their families. Maintain ongoing healthy communication with the patient\'s families
Accurately share information in team meetings
Facilitate referrals to ancillary services. Ensure the correct documentation is collected and/or received
Assist in ACA/AMA interventions with the Clinical and Nursing department
Maintain a basic understanding of health insurance benefits, health insurance terms, and other payment methods that are used for our organization
Complete all assigned documentation in a neat, accurate and timely manner
Exercise sound professional judgment and seek assistance as necessary
Protect the privacy of all patient information in accordance with the organization\'s privacy policies, procedures, and practices, as required by federal and state law, and in accordance with general principle of professionalism as a health care provider
Provide crisis support to other clinicians and patients, as needed and in emergency situations
Essential Functions For patient care needs, this is an on-site role. Meets and communicates with providers and partners, clearly deliver information related to continuing care services. Occasional driving to appointments. Consistent computer and phone, general office equipment use.
Qualifications A bachelor\'s degree from an accredited college and one year (1) of full-time satisfactory experience performing assessment and/or treatment of drug or alcohol addiction care OR a four-year high school diploma or its educational equivalent and two years (2) of full-time satisfactory experience in addiction care. CASAC, MSW and/or MHC preferred
Familiarity with FMLA and short-term disability processes, or a willingness to learn quickly
Familiarity with all relevant healthcare standards: OASAS, ASAM, JCAHO, etc.
Demonstrated communication, critical thinking, and interpersonal skills
Proficiency with data management and electronic health records
Strong problem solving, conceptual, and analytical skills
Preferred Qualifications Specific experience with stabilization and withdrawal management care and/or inpatient rehabilitation settings
Experience working as part of a multidisciplinary team
Knowledge of resources and different pathways of recovery for patients with substance use disorder
