The Case Manager works under the supervision of the Director of Behavioral/ Mental Health. They assist with assessing patient needs and determine how they can be helped with a focus on reducing recidivism. The Case Manager contributes to a treatment plan, advises patients about resources, services, and programs, sets up interviews to establish a need, and follows up to ensure the services are provided and that patient has completed referral process.

Essential Functions

Participates and assists in the vocational, educational, and social adjustment planning for inmates. If needed, enroll patients in appropriate community programs.

Maintains knowledge of community resources for behavioral healthcare and social services. Represents organization in professional manner in community and builds relationships with other community programs.

Provides pre-release counseling; researches and identifies community resources and services prior to processing offenders for release. Develops individualized plans for care and release.

Serves as liaison/patient advocate with program services (e.g., families, classification, medical, security, and other departments) for patients with medical or mental health needs.

Assists with scheduling patients with our providers and specialists when indicated, assists in facilitation of appointments when necessary.

Prepare educational materials as needed or required, using approved sources.

Contributes to the success of the organization towards the well-being of all inmates at the Marion County Jail.

Efficiently manage walk-ins and unscheduled sessions during the workday.

Communicate and interact with other staff on the jail medical care team effectively.

Document encounters and other services accurately and timely.

Demonstrates knowledge and understanding of patient privacy rights.

Maintains confidentiality of all medical, financial, and other sensitive materials and information in printed, electronic or verbal form, which may jeopardize the privacy of patients and/or employees.

Demonstrates accountability and responsibility for own work.

Ensures that treatment is delivered in accordance with the highest ethical and professional standards.

Ensures that patients’ rights are protected and report any abuse/neglect to the proper authorities.

Maintains knowledge of organizational policies, procedures and processes and provides accurate and clear information to patients.

Assists in the implementation of Quality Improvement/Quality Management (QI/QM) programs and the monitoring of compliance with required policies, procedures and standards with a commitment to service excellence and quality improvement.

Other duties as assigned.

Qualifications, Education And Experience

Bachelor’s degree in Social Work, Counseling, Psychology, or an acceptable related field.

Able to pass Level 2 background check.

Minimum five (5) years of experience providing behavioral health direct care.

Experience working in a corrections or lock-down facility.

Bilingual (English/Spanish) preferred.

Physical Demands
The Physical Demands describe essential functions: stand, walk, sit, use hands to finger, handle or feel objects, tools or controls, reach with hands and arms, climb stairs, balance, stoop, kneel, crouch or crawl, talk or hear, taste or smell. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Work Environment
Services are provided in the Marion County jail. Public safety and security are primary. All staff receive specific training in security and correctional procedures and must follow security measures. Work schedules may include evening or weekend hours.
