The Case Manager works in coordination with partner agencies to address client needs through referrals to providers and housing assistance. Responsibilities may include coordinated entry, outreach, prevention and risk reduction, crisis intervention, health education, referrals for substance abuse and mental health counseling, and benefits counseling. The Case Manager collaborates with program staff to ensure funds are used in accordance with grant requirements and maintains quality assurance of client files. Routine duties include interacting with school districts, police, government agencies, faith-based organizations, local businesses and nonprofit agencies to provide client services.
Compensation and Benefits
Pay Range: $23.00 - $28.00 per hour
40 hours/week – Full-time, non-exempt
Eligibility to enroll in benefits on the 1st day of the month following two months from date of hire

Basic Duties
Program Implementation: Assist clients with obtaining resources, developing case plans, and completing benefit applications and housing paperwork; advocate for clients; facilitate expedited service delivery through linkage activities
Case Management
Assess client readiness for case management services; create case plans with short, medium, and long-term goals
Assist clients with savings and budget planning; develop benefit enrollment applications
Prepare case-related reports on outcomes, successes, and challenges
Develop flexible, adaptable case management standards and establish collaborative relationships with community resources
Provide relevant referrals to agencies and service providers aligned with client goals
Advocate for clients with agencies and follow up to ensure timely access to services
Generate reports and information helpful to program activities
Outreach and Relationship Management
Work with supervisors to identify and engage housing opportunities through landlords, housing providers, and inventories
Engage clients in various settings, including unsheltered locations, and coordinate with partners to address trust, mental health, and substance use needs
Network with agencies, coalitions, law enforcement, local businesses, education, and community groups
Participate in staff meetings and training
Qualifications
Minimum of an Associate Degree in a related field or 2 years of related Case Management experience
Experience in nonprofit management, case management, and/or program development
Experience with social services and/or homeless programs
Proficiency in Microsoft Office, especially Excel
Self-motivated, with ability to develop creative solutions
Professional demeanor with strong written and oral communication, interpersonal skills, and teamwork abilities
Regular attendance and ability to multi-task in demanding situations
Customer-service oriented; able to interact with diverse individuals in office and field settings
Willingness to meet in offices and in unsheltered locations; able to tolerate outdoor conditions
Ability to stand, walk, bend, lift up to 20 lbs, and operate a computer; may require working in varied environments
Ability to adapt hours as job activities require, including evenings and weekends
Bilingual Spanish preferred

Successful completion of background screening
Reliable transportation, valid California driver’s license, and proof of auto insurance; must qualify for employer insurance
Current tuberculosis test
CPR/First Aid Certification
Work Environment
The work environment can involve contact with individuals and families in crisis, some of whom may have health or hygiene challenges. The role may require responding quickly to crisis situations and working in settings that may be outdoors or in environments with uneven terrain.
