Job Summary Marshall St. Landing Case Managers provide home-based case management services to families and individuals living at Family Tree’s Permanent Supportive Housing Program (PSH) in Arvada, Colorado. The program serves an 85‑unit PSH complex that houses people experiencing chronic homelessness or living at or below 30% of the area median income. Case managers work in clients’ homes, offering rental assistance, supportive services, resource connections, and goal‑setting within a Housing First framework.
Responsibilities Adhere to Family Tree policies, including mandatory reporting of child abuse/neglect and client confidentiality.
Provide and document home‑based case management per program guidelines.
Collaborate with outreach to identify and match clients to housing.
Assist clients with housing navigation, benefits acquisition, education/vocational referrals, financial empowerment, behavioral/health referrals, goal setting, and building a personal support network.
Develop, monitor, and revise housing stabilization plans with participants.
Screen and assess eligibility for homelessness programs, enter documentation into HMIS and reports for grant compliance.
Maintain an adequate case load, ensuring comprehensive client records.
Expectations Participate in monthly supervision (phone or in‑person).
Attend weekly staff meetings, supervision, on‑site/off‑site trainings, and other meetings as requested.
Maintain a valid Colorado driver’s license, car insurance, and clean motor vehicle record.
Ensure a safe work environment following all safety procedures.
Schedule Tuesday: 12pm‑8pm
Wednesday: 12pm‑8pm
Thursday: 12pm‑8pm
Friday: 2pm‑10pm
Saturday: 2pm‑10pm
Required Skills/Competencies Initiative – Work independently while communicating effectively within a team.
Relationship Management – Experience with under‑resourced, at‑risk families and individuals.
Flexibility and Adaptability – Learn quickly and navigate change well.
Strengths‑Based Approaches – Practice trauma‑informed, culturally appropriate, strengths‑based case management.
Teamwork and Collaboration – Build and maintain relationships with individuals, organizations, and team members.
Preferred Competencies Resourcefulness – Knowledge of community resources and voucher/Medicaid billing processes.
Digital Literacy – Proficiency with Microsoft Suite and HMIS.
Working Environment Reliable transportation required for off‑site meetings.
Physical Activities Talking, hearing (ordinary conversation)
Seeing (reading paperwork, computer work)
Manual dexterity sufficient to operate a computer, calculator, and telephone.
Physically exert force of 10‑15 lbs.
