Case Manager
Join Encompass Health as a Case Manager and play a vital role in coordinating exceptional post-acute rehabilitation care. This position places you at the heart of patient care, managing resources, coordinating interdisciplinary care plans, and ensuring seamless transitions from admission to discharge.
Here’s What You’ll Do
Work with an interdisciplinary team to develop and adjust treatment plans based on patient needs.
Coordinate tentative discharge plans, ensuring timely communication with families and providers.
Lead weekly team conferences and oversee all modifications to treatment plans.
Maintain accurate case management documentation and ensure compliance with all regulations.
Analyze Key Care indicators and conduct utilization reviews to improve quality and efficiency.
Participate in the case management on‑call schedule as needed.
Ensure accurate discharge and payer related information and facilitate post‑discharge referrals.

Qualifications
Licensed or certified professional in RN, SW, OT, PT, ST, Rehabilitation Counseling, or other relevant disciplines.
Active license if required by state regulations.
CCM or ACM certification required upon entry or to be obtained within two years.
Minimum associate degree in Nursing (RN) or bachelor's degree in other health care fields.
Preferred: 2+ years of rehabilitation experience.
