$16.00/hr - $20.00/hr

The Case Manager is responsible for supporting individuals and families who are unhoused or experiencing a housing crisis by providing case management services focused on housing stability and placement. The case manager connects individuals and families to community resources, enrolls clients from the Coordinated Entry System into HRI programs, develops partnerships with landlords and program partners, provides information and referral, facilitates crisis intervention, negotiates lease agreements, coordinates housing services, and strives to divert clients from emergency shelters.

Principal Accountabilities

Meet with clients at least once monthly or more frequently as required, usually in the client’s home.

Conduct assessments of clients’ housing environment for safety, lease compliance, and other needs.

Develop individualized housing and success plans.

Work outside of the office, in the community, for most scheduled work hours.

Participate in case conferences, team and community meetings.

Develop and foster landlord relationships, marketing HRI programs, and provide conflict resolution.

Provide direct service assistance ensuring shelter diversion or smooth transition from emergency shelter to affordable housing.

Complete calculations, forms, and documentation related to eligibility and enrollment.

Assist rent‑burdened families by negotiating lease addendums.

Conduct required housing unit inspections following all regulations.

Respond promptly to phone calls, emails, and visits within 2 business days.

Maintain active knowledge of all HRI programs, eligibility requirements, and available services.

Partner with human‑service providers and property owners for referral and supportive services.

Serve as the Agency’s Fair Housing representative when assigned.

Document all client and agency activities accurately within 2 business days.

Communicate information, decisions, and changes to clients in a formal, professional manner.

Assist with data collection for reporting and program development.

Review and comply with all policies, regulations, laws, and agency objectives.

Act with compassion, empathy, and care for people experiencing homelessness and housing crisis.

Maintain confidentiality and privacy of client and agency information at all times.

Position Specifications/Scope

Minimum Education: High School Diploma required; Bachelor's degree in social work or related field preferred.

Minimum Experience: Minimum of 2 years in case management; lived experience with homelessness or housing crisis preferred.
