Case Manager at HSS - Human Support Services

Role Overview
We are seeking a full‑time Case Manager to join our premier team of mental health professionals. The position supports clients at our supported residential housing program in Monroe County, IL, delivering comprehensive case management, advocacy, and therapeutic services.

Responsibilities

Provide case management and advocacy for clients.

Refer and link clients to community support services.

Conduct community education and outreach.

Engage clients using community and resource knowledge.

Document clinical services and maintain accurate records.

Meet agency standards for billable services.

Utilize a variety of therapeutic interventions to address clinical needs.

Complete required data entry and paperwork in accordance with program and agency policies and procedures, including opening requirements, assessments, treatment/recovery plans, progress notes, treatment authorizations, and authorization of medical necessity.

Qualifications

Bachelor’s degree in a Human Services field (Psychology, Social Work, Counseling, etc.).

Preferred: One year of relevant case‑management experience.

Ability to work in a multidisciplinary team environment.

Self‑starter with strong time‑management skills.

Valid driver’s license and insurance coverage for auto.
