Overview Position: Case Manager
Location: Colorado Springs, CO (In-person)
Employment Type: Full-Time
Schedule: 40 hours per week, schedule set by supervisor
The case manager works as part of a multi-disciplinary treatment team, serving as the primary contact for the client and the client’s service providers. Responsibilities include identifying barriers to access to mental health and substance use treatment, conducting functional assessments, and developing or enhancing stability in targeted areas of need. Under clinical direction, the case manager addresses skill and resource deficits and helps clients build competencies such as life skills, conflict resolution, self-advocacy, housing access, medical care, and job-seeking. The role involves collaborating with internal and external treatment team members to ensure continuity of care and linking clients with necessary resources. Work settings include outpatient, home, and community-based environments.
Essential Functions Completes functional needs assessment to assess resource barriers and skill deficits; contributes to treatment plan development based on assessments.
Enhances engagement with the organization throughout the client experience.
Collaborates with clients, internal and external partners, families, or other supports to coordinate care.
Facilitates individual and/or group sessions to enhance skills and resource access.
Evaluates client progress and provides feedback to the client, family, and treatment team.
Population served may include offender-based populations, first episode psychosis, inpatient psychiatry, substance abuse clients, depending on program assignment.
Maintains accurate, concise, and timely documentation of services provided.
Operates a personal vehicle as required to travel between offices or community sites; transportation of clients may be required using Diversus Health vehicles.
The environment at Diversus Health is fluid; roles and responsibilities may be altered to meet changing business conditions. Overtime may be required.
Knowledge/Skills/Abilities Bachelor’s Degree in Psychology, Sociology, Social Work or related field required.
One year of professional work experience or volunteer experience in the behavioral health field preferred.
Working knowledge of basic coping skills and community resources; general understanding of behavioral health and/or substance use services.
Ability to take initiative, organize work effectively, and use critical thinking to problem solve.
Excellent verbal and written communication skills; ability to build rapport with clients and team members.
Proficient computer skills, including Microsoft Suite; ability to type 25+ words per minute for input/output of client information using electronic health records.
