Case Manager at Veritas Management Group
The primary purpose of this position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations. The goal is to ensure that the medically related emotional and social needs of each resident are met and maintained on an individual basis.
Job Functions Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of the facility.
Assist in the development, administration, and coordination of department policies and procedures.
Keep abreast of current federal and state regulations, professional standards, and recommend policy changes to the Director or Administrator.
Review departmental policies annually and participate in recommended changes.
Develop policies for identifying medically related social and emotional needs of residents.
Participate in community planning related to the facility’s services and resident families.
Participate in discharge planning and development of social care plans and resident assessments.
Interview residents/families to obtain social history.
Perform administrative tasks such as completing necessary forms and reports.
Involve residents/families in planning social service programs when possible.
Arrange transportation to other facilities as necessary.
Refer residents/families to appropriate social service agencies.
Provide information about Medicare/Medicaid and other financial assistance programs.
Consult with staff, community agencies, and others to solve resident needs through program development.
Assist in reviewing and updating departmental job descriptions annually.
Assume authority, responsibility, and accountability of directing the social service department in the absence of the Director.
Maintain quality working relationships with medical professionals and other health related facilities.
Review and evaluate the department’s workforce and make recommendations to the Director.
Coordinate social service activities with other departments.
Implement recommended changes from facility consultants.
Delegate authority, responsibility, and accountability to other department personnel.
Make written and oral reports to the Director concerning department operations.
Assist in standardizing work methods.
Assist in providing solutions for social and practical environmental problems, including financial assistance and discharge planning.
Evaluate social and family information to assist in determining plans for social treatment.
Interpret social, psychological, and emotional needs of residents/families to the medical staff.
Obtain resources from community social, health, and welfare agencies.
Assist Infection Control Coordinator with social function classification.
Review personnel complaints and provide written reports to the Administrator.
Assist Quality Assurance Committee in developing action plans.
Maintain ongoing quality assurance program for social services.
Participate in facility inspections by authorized agencies.
Ensure charted progress notes accurately reflect services and resident response.
Maintain reference library of written material, laws, and regulations.
Involve residents/families in planning objectives and goals.
Meet with administration and related departments to plan social service activities.
Perform other duties as assigned.
Experience Minimum 1 year supervised social work experience in a health care setting, working directly with individuals.
Specific Requirements Must read, write, speak, and understand English.
Must make independent decisions when warranted.
Must handle personnel, residents, families, visitors, government agencies, and the public tactfully.
Must possess leadership ability and willingness to supervise other personnel.
Must have patience, tact, cheerful disposition, and enthusiasm for resident interactions.
Must seek new methods and incorporate them into services.
Must be a member in good standing of the National Association of Social Workers and Academy of Certified Social Workers, Inc.
Must be able to convey resident condition information accurately.
Must not pose a direct threat to health or safety of others in the workplace.
