The Position
Case Manager – responsible for coordinating the Individual Service Plans for Youth, helping families achieve stability in their homes and community.

Compensation and Benefits
Annual salary: $68,640 (base, depending on experience). Benefits include:

Qualifications
Knowledge of local community social services
BSW or equivalent degree in education, psychology, sociology, or related field
Bilingual in English and Spanish or ability to access professional interpretation services
Preferred experience with immigrant populations, family preservation, kinship care, and/or child welfare
Proficiency in Microsoft Office Suite and other office software
Superior oral and written communication skills and interpersonal skills
Detail oriented and organized
Ability to work independently and manage multiple projects with minimal supervision
Team player who maintains confidentiality and professional boundaries
Professional representation of the agency with all constituencies
Must pass pre‑employment health exam, TB and drug screen
Ability to drive to various sites and lift 10 pounds for filing
Insurability under corporate automobile insurance
Key Responsibilities
Maintain a caseload of Level 1 and Level 2 youth in the community
Support youth and sponsors in identifying strengths, needs, and community resources for stability
Complete appropriate documentation and implement Individual Service Plans
Ensure case‑management documentation in the UC Portal meets required timelines
Use professional judgment while maintaining a team approach to case planning
Provide crisis intervention and client de‑escalation as needed
Liaise with youth, families, sponsors, and other stakeholders
Perform other related duties as assigned
