CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Job Summary
The Case Manager provides community-based case management services to patients experiencing serious mental illness and/or those considered high risk due to their acuity level. Responsibilities include planning, connecting, advocacy, coordination, and monitoring to help patients access health services, financial assistance, housing, employment, education, social services, and natural supports. The role involves assessing needs, developing and monitoring individualized treatment plans, and documenting services accurately and promptly. The Case Manager promotes a recovery-oriented environment and employs advanced techniques to serve high-risk individuals in their program.
Skills/Requirements
High school diploma or GED required; Associate's Degree or higher in a related behavioral health field preferred.
Recovery from alcohol or drugs and/or GMH for at least 1 year preferred.
Minimum of 1 year experience in a behavioral health role involving outreach and engagement activities.
Equivalent combination of education and experience acceptable.
Current AZ Driver's License and 39-month MVR.
Behavioral Health Technician (BHT) certification per A.A.C. R9-10-101.33 and CBI policies.
Arizona Fingerprint Clearance Card.
Jail Clearance (if applicable).
