Case Manager
Position at Singing River Health System.
Position Overview : The Case Manager focuses on clinical quality and resource management, resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care for a population of various ages within the Singing River Health System. All duties are performed in accordance with SRHS procedures and policies, accreditation organization standards, and governing guidance and publications for health care employees.
Key Responsibilities :
Work effectively under pressure, multitask, and be self‑directed.
Demonstrate strong written and oral communication, analytical and critical thinking, conflict management, team‑building, and organizational skills.
Perform numerous tasks simultaneously with minimal errors.
Participate in daily discharge planning meetings, addressing barriers that may inhibit discharge and targeting successful transitions between levels of care.
Maintain seamless communication between the inpatient care management team and the outpatient case management team.

Education : Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
License : Current unrestricted Mississippi State licensure as a nurse.
Certification : Must complete and maintain BCLS certification upon hire and/or transfer; maintain TB certification as applicable; complete de‑escalation training by 90 days.
Experience : Two (2) years of nursing experience in ambulatory or acute care with relevance to case management.
Reports To : Director – Patient Resource Management.
Physical Demands : Moderately active work involving sitting, moving, and occasional travel within the SRHS service area. Requires lifting, carrying, and repetitive use of office equipment. Adequate visual and auditory acuity required.
Mental Demands : Requires keen mental faculties, superior communication skills, emotional stability, and the ability to work under pressure and meet deadlines. Attention to detail and multi‑tasking in complex situations are essential.
Special Demands : Superior customer service skills, professional etiquette, and proficiency with computer technology (Outlook, Word). Demonstrate appropriate clinical judgment for a patient population of all ages.
