Job Summary
The Case Manager provides support and advocacy to clients and their families within the community to educate and promote healthy lifestyles and prevent reoccurring hospitalizations by monitoring, assessing, advocating, supporting and empowering members to improve their quality of life through maintaining stability in the community.
Responsibilities
Conduct initial and annual assessments.
Provide population and age‑specific case management services to members.
Assess the personal needs of each member.
Participate with members in the development of the Individual Plan of Service.
Provide support to members and families in identifying and accessing resources.
Maintain required case management documentation.
Participate in in‑service training, clinical supervision and consultations.
Participate in group meetings related to substance abuse and mental health.
Assist in providing education to non‑CMH providers regarding mental health.
Assist in the development of community contacts and strengthen networks.
Education & Experience
Bachelor’s degree in human services required.
One year of experience in working with a person who is exhibiting mental illness.
Valid Michigan driver’s license required.
Must complete requirements for a Michigan Certified Board of Addiction.
Must be willing to complete requirements for Professionals of MBAP.
Basic Life Support (BLS) certification required.
