Position Summary
The Case Manager provides support using a trauma-informed, person-centered approach to individuals in substance use and/or mental health recovery with a history of homelessness. During the program’s start-up phase, the Case Manager supports individuals as they transition from homelessness into their own studio apartments within a congregate supportive housing site. Once housed, the Case Manager continues to provide ongoing support to help participants achieve their goals—stabilizing income, strengthening coping skills, accessing medical care, pursuing employment, and more.
Pay Rate: $20 an hour
Open Shift: Monday through Friday 9am‑5pm, some flexibility required
Key Responsibilities
Coordinate all intake documentation and complete required assessments and service plans for program entry.
Collaborate with multidisciplinary teams to develop and implement individualized action and safety plans tailored to participant needs.
Assist participants in transitioning to and maintaining stable housing, including communication with property management and understanding housing responsibilities.
Support participants in obtaining and managing income, employment, vocational training, and public benefits.
Encourage community engagement and support skill development in areas such as daily living, money management, and social participation.
Provide emotional support, coordinate health-related services, and assist with medical, dental, and psychiatric appointments.
Maintain strong presence in the community by working directly with participants and supporting access to necessary resources.
Participate in supervision, team meetings, and ongoing training to support professional growth and quality service delivery.
Provide transportation when needed and ensure safety standards are met, including proper documentation of any incidents or concerns.
Use agency systems effectively for communication, documentation, and reporting, while following all safety and compliance protocols.
Requirements
Valid driver’s license, adequate automobile insurance, and reliable transportation.
Ability to work independently and as part of a team.
Strong organizational, communication, and interpersonal skills.
Must be at least 21 years of age.
Able to obtain/maintain CPR and First Aid certifications.
High school diploma or equivalent required.
Minimum of six months of relevant experience required.
Experience working with individuals with mental health/substance use challenges or housing instability preferred.
