About CPLC LUCES
CPLC LUCES (Latinos Unidos Contra El Sida/Latinos United Against Aid) provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.
Community‑Based Role
The case manager engages with clients in different settings, including office, homes, off‑site community and/or clinical locations. This role also attends primary medical care appointments with clients as needed. The case manager travels throughout Maricopa and Pinal County, utilizing an insured personal vehicle.
About the Role
Under general supervision, the case manager provides direct covered services to individual clients, coordinates with community support and stakeholders, manages appointment schedule and caseload, refers clients to medical services, conducts needs assessments, implements treatment plans, facilitates referrals, manages the clinical care team, and participates in ongoing learning experiences.
What Makes a Great Supportive Case Manager?
Non‑judgmental, unconditional support to historically marginalized communities, especially Spanish‑speaking and LGBTQIA+ communities.
Knowledge of the dynamics of HIV, family violence, substance abuse and the ability to refer clients to appropriate resources.
Experience with psychosocial assessments.
Knowledge of the Ryan White healthcare system.
Skillful navigation of community resources and support for clients in securing services.
Minimum Qualifications
Bachelor’s degree from an accredited college in a licensed field related to case management such as social work, nursing, public health, or other human services related field.
or high school diploma plus 4 years of comparable professional knowledge, skills and abilities specific to supportive case management may substitute for the degree.
Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and other STDs.
Bilingual: English/Spanish skills.
Valid driver’s license and automobile insurance.
