Encompass Health is seeking a dedicated Case Manager to join our team. As a leading provider of rehabilitation care, you will serve as a critical link between exceptional clinical support and transformative patient outcomes.
Responsibilities
Work with an interdisciplinary team to guide treatment plans based on patient needs and preferences.
Coordinate with the team to establish tentative discharge plans and contingency plans.
Participate in planning and execution of patient discharge experiences.
Monitor patient experience: quality, timeliness, service appropriateness, payer expectations.
Facilitate weekly team conferences and coordinate all treatment‑plan modifications.
Complete case‑management addendums and required documentation.
Maintain knowledge of regulations, standards, company policies, and departmental operations.
Review and analyze case‑management reports, including Key Care Indicators, and plan appropriate actions.
Understand commercial contract levels, exclusions, payer requirements, and recertification needs.
Attend Acute Care Transfer (ACT) meetings to identify trends and collaborate on reducing ACTs.
Meet with patients/families per the Patient Arrival and Initial Visit Standard within 24 hrs of admission.
Perform assessment of goals and complete case‑management addendum within 48 hrs of admission.
Educate patients/families on rehabilitation and the Case Manager role; establish communication plans.
Schedule and facilitate family conferences as needed.
Assist patients with timely procurement/planning of resources to avoid discharge delays.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate, timely referrals, including documentation to post‑discharge providers/physicians.
Ensure accuracy of discharge and payer‑related information in the patient record.
Participate in utilization review: data collection, trend review, and resolution actions.
Participate in the case‑management on‑call schedule as required.
Qualifications
License or Certification :
Must be qualified to independently complete an assessment within the scope of practice of your discipline (RN, SW, OT, PT, ST, or Rehabilitation Counseling).
If licensure is required for your discipline in your state, you must hold an active license.
Must meet eligibility requirements for CCM® or ACM™ certification upon entry or obtain it within two years.
Minimum Qualifications :
For Nursing: associate degree in nursing (RN), BSN preferred, and state-approved licensing.
For other licensed or certified health care professionals: minimum bachelor’s degree; graduate degree preferred.
Preferred: 2 years of rehabilitation experience.
