Base pay range $70,000.00/yr - $90,000.00/yr
Job Description As a Case Manager at Encompass Health, you will be a vital link between exceptional care and the transformative impact on each patient's journey. Your responsibilities will include:
Guiding treatment plans based on patient needs and preferences.
Coordinating with the interdisciplinary team to establish tentative discharge plans and contingency plans.
Participating in planning for and the execution of patient discharge experience.
Monitoring patient experience: quality, timeliness, service appropriateness, payors, and expectations.
Facilitating team conferences weekly and coordinating all treatment plan modifications.
Completing case management addendums and all required documentation.
Maintaining knowledge of regulations, standards, company policies, and department operations.
Reviewing and analyzing case management reports, including Key Care Indicators, and planning appropriate actions.
Understanding commercial contract levels, exclusions, payor requirements, and recertification needs.

Qualifications To be qualified for this role, you must:
Possess a minimum of an Associate Degree in Nursing, with a BSN preferred, or a bachelor's degree in a related field.
Have 2 years of rehabilitation experience preferred.
Be qualified to independently complete an assessment within the scope of practice of your discipline.
Meet eligibility requirements for CCM or ACM certification upon entry into this position or within two years of entry.
