Join the Community Bridges team as a Case Manager in Phoenix, AZ. We provide high‑quality behavioral health services across Arizona, including residential, outpatient, and crisis care.

Job Summary
The Case Manager (CM) delivers case management services in community or office settings. The CM assists patients and their families in planning, connecting, reaching out, advocating, coordinating, and monitoring the patient’s needs and progress. The CM helps patients access health services, financial assistance, employment, education, social services, and natural supports. The CM assesses patient needs, develops and monitors individualized treatment plans, and documents services accurately and timely. The CM embraces a recovery‑philosophy and promotes a safe recovery environment for clients.

Responsibilities

Provide case management services in community or office settings.

Plan, connect, outreach, advocate, coordinate, and monitor patient’s needs and progress.

Assist patients in accessing health services, financial assistance, employment, education, social services, and natural supports.

Assess patient needs and develop individualized treatment plans.

Document services provided accurately and timely.

Promote a safe recovery environment and embrace a recovery‑philosophy.

Qualifications

High school diploma or GED required; an associate’s degree or higher in a behavioral health field preferred.

1‑3 years of experience working with the GMH/SU population.

1‑3 years of experience in a behavioral health position providing outreach and engagement activities.

1‑3 years of experience in a medical setting preferred.

Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred.

Current AZ Driver’s License valid and good standing.

Clear 39‑month Motor Vehicle Record.

Arizona Fingerprint Clearance card or good cause exemption required.
