Responsibilities
Assess patients’ health conditions and needs in their homes to develop personalized care plans.
Coordinate with physicians, nurses, and other healthcare professionals to implement care plans and ensure continuity of care.
Educate patients and their families on managing illnesses or injuries and guide them on using home health care equipment.
Monitor patients’ progress and adjust care plans as necessary to meet their changing health needs.
Facilitate access to community resources and services, such as support groups or home health aides, to assist patients and families.
Document patients’ care, including assessments, care plans, and progress, ensuring compliance with healthcare regulations and privacy laws.
Manage the distribution and administration of medication, including educating patients on proper usage and potential side effects.
Conduct home safety evaluations to identify potential hazards and recommend modifications to prevent injuries.
Qualifications
Bachelor's degree in health sciences, social work, psychology, or a related field preferred.
Prior experience in case management or similar role.
Communication, organizational, and problem‑solving skills.
Must hold a valid Nursing License in state.
