Case Manager – Yakima Valley Farm Workers Clinic, Toppenish, WA

Full-time, Non-exempt, No relocation.

Position Highlights:

DOE $24.77-$30.34, higher for experienced candidates

What You’ll Do:

Build program-specific caseload by seeking referrals from providers and partners.

Conduct intake screenings to assess clients, identify immediate needs, and determine appropriate YVFWC case management services.

Perform in-depth client interviews (in-person) using systematic social services assessment to determine status, barriers, and needs.

Refer clients to appropriate agencies/services based on determined needs.

Assist clients with self-referral process.

Regularly meet with clients (often in-home) to monitor service plan implementation, advocate, and resolve problems.

Maintain each client’s service plan and documentation in accordance with program guidelines and regulatory requirements.

Establish and maintain relationships with other service providers/ agencies; stay updated on community services.

Participate in interdisciplinary team case reviews and other meetings as need.

Perform clinical responsibilities in alignment with Joint Commission, HRSA, and YVFWC requirements.

Qualifications:

Bachelor’s degree in Social Work, Behavioral Sciences, Psychology, or Child Development.

Associate’s degree with two years’ experience in community social services, public health, crisis intervention, outreach or linkage programs, OR

High school or GED with four years’ experience in community social services, public health, crisis intervention, outreach or linkage programs.

Valid driver’s license and proof of vehicle liability insurance coverage.

Bilingual (English/Spanish) preferred; must demonstrate level 10 language proficiency for differential pay.

Effective verbal, written, and listening communication skills.

Knowledge of medical terminology and local social and health services.

Ability to develop effective relationships with social service providers and partners.

Basic proficiency with Microsoft Outlook, Word, and Excel.
