The Case Manager is responsib le for providing outreach programs and activities, provid ing social support and counseling to assigned clients, advocat ing for clients and follow ing -up with assigned client ( s ) health management/care plans .
Posted pay range is the starting salary. Pay rate offered is based on experience.
Provide s case management to client population .
Partners with client(s) to identify needs, agree on goals, choose treatment methods, and articulate specific treatment outcomes and objecti v es.
Coach es client(s) in effective management of their chronic health conditions and self-care
Assist s clients in accessing health related services including obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services
Act s as an advocate and liaison between the client /family, provider, health plan and community service agencies. Assists clients’ with scheduling appointments and assisting with completion of applications for programs for which they may be eligible.
Document s all client encounters including contact information for both client and relevant contacts related to client care.
Build s and maintain s positive working relationships with clients, providers and community service agencies; work collaboratively and effectively as part of a team.
High School Diploma or equivalent
One ( 1 ) year of experience in a health and/or social services setting
Preferred: Bachelor’s degree in social work, psychology, counseling, public health or related field
Valid Texas Driver’s License
Community Health Worker (CHW) C ertification within 6 months of hire
