North American Family Institute (NAFI) is hiring a full-time Case Manager to support adolescent youth and families involved with the Florida Department of Juvenile Justice. This position is based out of our Jacksonville office and focuses on providing case management services, assisting a Family Therapist, and conducting visits to residential programs.
Key Responsibilities:
Provide direct case management services to youth and families
Assist the Family Therapist in service delivery
Conduct visits to residential facilities and community settings
Transport clients as needed
Maintain accurate records and documentation in electronic health record systems
Requirements:
Bachelor’s Degree in a related human services field
Experience working with juvenile justice or at-risk youth
Strong teamwork and communication skills
Valid Florida driver’s license, clean driving record, and reliable transportation
Flexible schedule, including availability for some evening hours
Comfortable with computers, data entry, and electronic health records
Background screening, proof of highest education level, and auto insurance are required prior to hire
Compensation for this position is $18.50 per hour ($38,480 a year)
