Our client is seeking a dedicated Qualified Developmental Disabilities Professional (QDDP) to oversee the planning, coordination, and compliance-related supports for adults living in their licensed group homes. The QDDP will serve as the lead case manager for the 25‑30 individuals living in the homes. Full time with flexible hours within business needs.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
Develop, update and monitor person-centered individual support plans (ISPs)
Review and audit daily documentation
Submit CHRIS incident reports
Train and coach Direct Support Professionals (DSPs) and house managers
CLIENT'S REQUIRED AND EXPERIENCE AND EDUCATION
Bachelor's or Master's in Social Work, Psychology, Human Services, or related field
Minimum of 3 years of experience supporting individuals with ID/DD
Solid understanding of DBHDS licensing, CHRIS, person-centered planning and Medicaid Waiver documentation
