Overview Case Manager - Crisis Walk-In Center - Full Time - Montrose, CO
Position Summary: We are seeking a Case Manager who will participate as a member of the program team to design and implement the individual care plans for clients in accordance with policies, rules, regulations, and directives which apply. Utilizes a variety of treatment approaches and case coordination to work with a diverse client population. This position will provide service coordination services for clients as assigned.
Salary: Starting at $19.50 (Base $18.50 + $1.00 Locale differential) - $22.22 per hour (non-exempt/hourly) and considers preferred experience and preferred education described below.
Responsibilities The Case Manager will participate as a member of the program team to design and implement individual care plans and provide service coordination for clients as assigned, using multiple treatment approaches and coordinating with a diverse client population, in accordance with applicable policies, rules, regulations, and directives.
Qualifications Our ideal candidate for the Case Manager position will:
Maintain a bachelor’s degree in one of the recognized mental health professions.
Have previous experience with substance abuse and mental health related case management and treatment.
A post graduate degree (Masters) in one of the recognized mental health professions and two years human service-related experience are preferred.
Required Skills Knowledgable regarding patient health information and HIPAA compliance.
Organized with the ability to follow guidelines.
Excellent interpersonal and communication skills.
Ability to be self-directed and to work independently with minimal supervision; able to plan, multi-task, and prioritize in a fast-paced setting.
Ability to follow the organization’s confidentiality and security rules.
Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax and multi-line telephone.
Working knowledge of Windows applications including MS Word, MS Excel, MS Outlook and internet access.
Ability to develop and maintain effective working relationships with internal and external customers; communicate job-related information to individuals and small groups.
Respect for confidentiality and the ability to handle sensitive information.
Current driver’s license and ability to maintain insurance eligibility; occasional travel required.
Ability to take on additional duties when necessary.
