Job Title: Care Manager
Department: MI, Care Management Services, Clinical Services
Location: Ehardt/Children’s/Administrative/Croswell Site
Classification: Seven (7)
Summary: This position is responsible for all functions of case management including advocacy, coordination assessment, linking, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of developmental disability or mental illness. Settings include the CMH office, home, and/or the community. This position will perform the following duties.
Essential Duties and Responsibilities
Gathers and reviews information such as medical, psychological, and social history to evaluate needs and capabilities toward the least restrictive environment and/or services.
Initiates and coordinates professional assessments and shares assessment/treatment data as authorized for implementation and coordination with other agencies, courts, and caretakers.
Completes the person-centered plan of service and periodic progress reviews; develops short- and long-term goals; ensures plan reflects the individual’s preferences and coordinates services in residential, employment, school, and other settings.
Acts as primary advocate for assigned individuals and primary liaison with families, guardians, caretakers, and involved agencies.
Provides consultation, referral, and in-services to staff, caretakers, families, and other agencies as needed.
Participates in placement planning and completes forms for placement; coordinates residential treatment plans and conducts required home visits.
Provides community outreach to prevent crises for individuals with hospital recidivism history.
Participates in after-hours on-call, crisis intervention, and other duties as assigned.
Participates in evidence-based practices as assigned (DBT/IDDT/FPE/IPS, etc.).
Provides care management services to veterans and their families, as assigned.
Completes OBRA assessments and/or OBRA coordination as directed; supports Grant requirements as needed.
Ensures integrated health care needs are addressed and coordinates linkage to external services as applicable.
Performs initial intakes as assigned by Supervisor.
Job Function/Areas
Children’s Care Manager: Linking services (e.g., school, probation, DHHS); family psychoeducation; participate in BTC committee; work with populations 0-21 years with SED or IDD dx.
Adult Care Manager: Linking services (e.g., SS, probation, DHHS, MRS, jail services); family psychoeducation; participate in BTC committee.
Behavioral Care Manager: Linking and coordinating services; provide behavioral intervention support; complete behavioral assessments, FBA, and behavioral plans; set up data collection; provide progress summaries to providers and BTC.

Qualifications/Requirements
Bachelor’s degree in a Human Services field required; licensure as RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, or LPC preferred. SST licensure for Medicaid requirements. Care Manager-Behavioralist requires BCaBA. QMHP, QIDP, or CMHP qualification where appropriate.
Veteran status required for working with Veterans. For work with children, 40 hours of Children’s training per calendar year and CAFAS rater certification are required. Minimum three years of experience with examination, evaluation, and treatment of SED children and families.
Substance license preferred; consider applying for MCBAP development plan.

Physical Demands & Work Environment
The physical demands are those typical of office and community-based care management work. Reasonable accommodations may be provided. Travel between offices and to client sites may be required. Union and Benefits
Local 1518 union status; eligibility for medical and other benefits requires working over 30 hours per week. PTO accruals, holidays, union dues, tuition reimbursement, and retirement benefits are provided per the union contract and agency policy.
