Base Pay Range
$71,100.00/yr – $97,800.00/yr

Role Responsibilities

Primary advocate for beneficiaries and families; conduct assessments and develop comprehensive care plans (CCP), updating every 6 months; provide CCP copies to providers and parents/caregivers.

Serve as point of contact for medical and behavioral care coordination; ensure baseline measures for the Autism Care Demonstration (ACD) are completed and outcome data reported.

Perform telephonic care management across the East region, coordinating services for ASD treatment.

Assess beneficiary needs and collaborate with providers, caregivers, and stakeholders to initiate treatment and remove barriers.

Develop and maintain educational and community resource plans, including educational materials and support groups.

Use motivational interviewing and solution‑oriented approaches in communication.

Participate in integrated care plan meetings, coordinated team conferences, and serve as meeting facilitator when appropriate.

Communicate and coordinate with other Autism Care Navigators (ASNs) to ensure continuity of care during relocations.

Monitor and maintain ECHO registration and Enterprise Family Member Program (EFMP) enrollment for qualifying beneficiaries.

Ensure data entry accuracy and monitor reports for correct reporting.

Required Qualifications
Must meet ONE of the following license requirements:

Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW‑ACP), or Clinical Psychologist

Clinical experience in pediatrics, behavioral health, and/or ASD; healthcare environment; proven care‑management experience.

Active Certified Care Manager (CCM) designation (or obtain within first year).

Minimum 1 year post‑degree clinical experience.

Minimum 1 year managed‑care experience.

Licensed Professional Counselor (LPC), Licensed Clinical Marriage and Family Therapist (LCMFT), or Licensed Mental Health Counselor (LMHC)

Clinical experience in pediatrics, behavioral health, and/or ASD; healthcare environment; proven care‑management experience.

Active CCM designation.

Minimum 1 year post‑degree clinical experience.

Minimum 1 year managed‑care experience.

Registered Nurse (RN)

Clinical experience in pediatrics, behavioral health, and/or ASD; healthcare environment; proven care‑management experience.

Minimum 3 years clinical nursing experience.

Minimum 1 year managed‑care and case‑management experience.

Active CCM designation (obtain within first year if not present).

Must also meet ALL of the following requirements:

U.S. citizenship (Department of Defense contract).

Interim approval for government security clearance (eQIP).

Not authorized to work in Puerto Rico under the contract.

Proficiency in Microsoft Office (Word, Excel, Outlook).

Strong written and verbal communication skills.

Ability to handle high volume of calls and customer contacts politely and professionally.

Ability to manage multiple projects simultaneously and prioritize tasks.

Preferred Qualifications

Prior experience with the TRICARE Autism Care Demonstration.

Knowledge of applied behavior analysis and integrated care for autism.

Direct or indirect military experience preferred.

Extensive analytical skills.

Bilingual capability a plus.

Work At Home / Remote Requirements

Minimum download speed: 25 Mbps; minimum upload speed: 10 Mbps (wireless, cable, or DSL).

Satellite, cellular, or microwave connections only with leadership approval.

Calif., Illinois, Montana, or South Dakota residents receive a bi‑weekly internet payment.

Humana supplies telephone equipment as needed.

Dedicated, interruption‑free workspace protecting PHI/HIPAA information required.

Schedule & Training
Work Days/Hours: Monday‑Friday, 8:00 a.m. – 7:00 p.m. EST (8‑hour shift).
Training: Mandatory first 4‑6 weeks, 8:00 a.m. – 5:00 p.m. EST.

Travel: Occasional travel to Humana offices for training or meetings may be required.

Weekly Hours: 40

Compensation & Benefits
Starting base pay approximately $71,100 – $97,800 per year, subject to geographic and individual factors. Eligible for a bonus incentive plan based on performance.
