Overview Humana Healthy Horizons focuses on helping people achieve their best health. The individual in this role will work as an Oklahoma-based, primarily telephonic care manager, assessing and evaluating enrollees’ needs to achieve or maintain optimal wellness by guiding enrollees/families toward and facilitating interaction with appropriate care resources. This role works in collaboration with the interdisciplinary care management team to provide holistic, person-centered care. The team-based approach includes community health workers, housing support specialists, SDOH coordinators, and care management support assistants.
Responsibilities Include Employ a variety of strategies to manage enrollees’ behavioral, physical, environmental, and psycho-social health needs.
Monitor assessments and evaluations to ensure enrollees progress toward desired outcomes.
Identify and resolve barriers to effective care and continuously monitor progress toward outcomes.
Create enrollee care plans and align with department, segment, and organizational objectives.
Make decisions about work methods with minimal direction, seeking guidance when needed.
Perform telephonic and face-to-face assessments to guide members/families toward appropriate resources for care and overall wellbeing.
Collaborate with providers and community services to promote quality and cost-effective outcomes.
Follow established guidelines and procedures.
Potential travel to Humana Oklahoma office for meetings and training.
Required Qualifications Must reside in Oklahoma.
Active RN license, LPC, or LCSW.
Minimum 1 year clinical experience.
2 or more years of care management experience.
Experience with Medicaid and/or Medicare enrollees coordinating services or benefits.
Knowledge of community health and social service agencies.
Excellent communication and interpersonal skills with ability to build rapport.
Proficiency in Microsoft Office (Word, Excel, PowerPoint).
Valid state driver’s license and proof of personal vehicle liability insurance with 100/300/100 limits (Humana Driver Safety Program).
Participation in Humana TB screening program; must be screened for TB if selected.
Preferred Qualifications Case Management Certification (CCM).
Bilingual in Spanish.
Work at Home Guidance Home/Hybrid location with internet: minimum 25 Mbps download and 10 Mbps upload. Wired/wireless connections acceptable; satellite/cellular allowed only with leadership approval.
In some states, bi-weekly internet expense stipend may apply.
Humana provides appropriate equipment for remote work; maintain a dedicated space to protect member information.
Additional Information Workstyle: Remote, in-state Oklahoma.
Travel: Up to 10% in Oklahoma.
Core Workdays & Hours: Typically Monday – Friday, 8:00am – 5:00pm CST.
Interview Format We use HireVue for interviews. Selected candidates may participate in a pre-recorded voice interview and/or SMS text interview. Timings are typically 10–15 minutes for voice and 5–10 minutes for SMS. Interviews are reviewed to determine next steps. Travel to Humana offices for training or meetings may be required.

Pay Range: $71,100 - $97,800 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
