Care Manager LCSW – Medical Management is a role within Providence’s Care Management team that provides comprehensive, patient‑centered support in collaboration with physicians, nurses, other clinicians, and the community.
Description
Care Management is a collaborative practice model that includes patients, nurses, Care Manager LCSWs, physicians, other clinicians, caregivers and the community. The process relies on excellent verbal and written communication and effective resource coordination to address the psychological, social, and health needs of patients. The Care Manager LCSW advocates for the patient’s overall well‑being and works to facilitate access to care and appropriate use of resources. Core duties include assessment, planning, implementation, coordination, monitoring and evaluation of the plan of care, with management of resources and continuity of care performed in alignment with Providence’s mission, vision and core values.
Required Qualifications
Master's degree in Social Work
California Clinical Social Worker License upon hire
2 years of relevant Social Work experience in a medical group, health plan, hospital, community, post‑acute or medical office/clinic setting
Valid driver’s license and auto insurance; driving may be required for the role
Preferred Qualifications
3 years of Care Management and Managed Care experience in acute, ambulatory or community health setting
3 years of Behavioral Health experience in acute or outpatient setting
