Care Manager LCSW – Providence Little Company of Mary Medical Center San Pedro
Full‑time, 8‑hour day shift position. Providence Little Company of Mary Medical Center in San Pedro, CA, is accredited with the Joint Commission’s Gold Seal of Approval®, the American Heart Association/American Stroke Association’s Gold Plus Achievement Award, is nationally ranked by UDSMR, and CARF‑accredited for its outstanding rehabilitation programs.

Care Management within the Department of Psychiatry is a collaborative practice that includes patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. They recognize the patient’s right to self‑determination, the significance of social determinants of health, and the complexities of care—including barriers to coping, abuse, mental health, substance use, lack of resources, and psychosocial issues.

Required Qualifications

Master’s Degree in Social Work.

All LCSWs must meet the Los Angeles County Department of Mental Health guidelines for LPS designation and maintain current status.

California Clinical Social Worker License upon hire.

Within 18 months of hire, designated as a Behavioral Health Clinician to perform 5150 evaluations.

Within 1 year of hire, CPI certification (non‑violent crisis intervention training).

At least 2 years of experience in a healthcare setting with a psychiatric mental health patient population, preferably in an inpatient setting.

Computer proficiency.

Preferred Qualifications

Discharge planning experience.

Base pay range: $41.90 – $66.22 per hour. Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
