Overview
Monroe Plan for Medical Care is hiring Care Managers in the Albany area. As part of a dedicated, caring team, you will help improve access and quality of healthcare for underserved populations.

Position Summary
Provide care management services to individuals eligible for Health Home services. Work largely independently with guidance from a Team Lead or Supervisor as needed.

Essential Duties and Functions

Care Management (50%)

• Receive referrals and contact members promptly.

• Conduct comprehensive bio‑psycho‑social assessments.

• Build therapeutic relationships and develop person‑centered care plans.

• Coordinate services with health and community providers.

• Maintain minimum monthly telephonic contact and in‑person visits every three months.

• Travel for home visits and community activities as required.

• Adhere to Monroe Plan protocols and professional boundaries.

Documentation (30%)

• Complete accurate, clear, and timely documentation.

• Show proficiency in program software within 3 months of hire.

• Maintain comprehensive assessments and care plans per regulatory requirements.

• Update documentation as contacts and actions occur.

Collaboration (15%)

• Participate in multidisciplinary care teams.

• Facilitate member‑focused meetings and community provider collaboration.

• Support team coordination and cross‑coverage.

• Engage in community activities and in‑service training.

Communication (10%)

• Present professionally to build therapeutic relationships.

• Provide feedback to providers on member progress and barriers.

• Share program information with members and partners.

Other Functions and Responsibilities

Cannot perform licensed tasks such as diagnosing or prescribing medication.

Cannot transport active members or provide hands‑on care.

Minimum Requirements / Licenses / Certifications

Master’s degree (or equivalent) in Social Work, Psychology, Nursing, Rehabilitation, OT, PT, Counseling, or related field AND 1‑year experience working directly with SMI, developmental disabilities, alcoholism, or SED populations.

Or Bachelor’s degree with 2‑years of similar experience.

Credentialed Alcoholism and Substance Abuse Counselor (CASAC) preferred.

Demonstrate respect for diversity, confidentiality, teamwork, communication, organizational skills, and ability to work independently.

Computer literate.

Valid NYS driver’s license with reliable transportation for travel requirements.

Preferred Qualifications

Previous experience as a Health Home Care Manager.
